Cold plunge and contrast therapy: benefits, risks and evidence

Sauna, cold plunge and rest make up contrast therapy. Here is what the research shows about benefits and recovery, what is marketing, and which risks call for caution.

Sauna, cold water, rest. The cycle has become the program at many bathhouses and at nearly every recovery studio. It has also become a promise machine: de-inflame the body, boost immunity, “detox”, improve sleep, strengthen the mind, speed up recovery and, if the advertising is especially ambitious, change your life in forty minutes.

It is worth separating these things. Contrast therapy is a real and intense physical experience. There is serious research on part of it, mostly on recovery after exercise. There are reasonable hypotheses that have not been well tested. And there are benefits sold as certainties that the literature does not support.

What contrast therapy means

Broadly, it is alternating heat and cold with rest in between. In a bathhouse the heat is usually a dry or steam sauna; the cold is a plunge tub, a bath or a shower; rest happens between stages. The cycle can be repeated, but there is no universal protocol.

One important caveat: in the studies, “contrast therapy” generally means alternating hot and cold water immersion. That is not quite the same as stepping out of an 80 °C sauna into an ice tub. Both experiences share the logic of thermal change, but they should not be treated as equivalent when someone cites a research result.

In heat, the skin warms, peripheral vessels dilate and the body tries to shed heat. In cold, the opposite response: vasoconstriction, faster breathing, and a momentary rise in heart rate and blood pressure. That is physiology, not a metaphor about a “shock that cleanses the system”. A review of cold water immersion found clear cardiovascular, respiratory and metabolic changes, but also concluded that the mechanism behind athletic recovery remained uncertain.

What the research supports best: recovery after exercise

The most plausible and most studied benefit is a reduction in the sensation of muscle soreness and fatigue after hard exercise. A 2013 systematic review found better results from contrast therapy than from passive rest on some recovery outcomes. The authors themselves, however, flagged a high risk of bias in the included studies and noted little evidence that it beats other popular strategies.

The picture stayed mixed in more recent work. A 2024 meta-analysis placed contrast therapy well for recovering creatine kinase, a marker associated with muscle damage, but pointed to cryotherapy as the better option for delayed-onset soreness and neuromuscular recovery. Another review and meta-analysis concluded that cold water immersion on its own outperformed contrast therapy on several post-exercise outcomes.

In practice the honest conclusion is less spectacular: after a heavy session, contrast may leave some people feeling less sore or less tired than they would sitting still. That does not prove it rebuilds muscle faster, or that it improves performance in every kind of training. Sleep, food, training load and time remain the larger part of recovery.

Plenty of people leave the circuit relaxed, alert or with a quieter head. That experience is worth something on its own, but it should not become a clinical prescription. Research on cold bathing alone has found some signs of reduced stress hours after exposure and possible improvements in sleep and quality of life, but the 2025 review also highlights few trials, small samples and wide variation between protocols.

For the combination of sauna, cold and rest, robust studies isolating each stage and following people long enough are still missing. It is possible that the ritual, the distance from the phone, the pause in the calendar and the company matter as much as the water in how good you feel afterwards. That does not diminish the experience. It just changes how to talk about it.

Circulation and cardiovascular health

Heat and cold temporarily change the calibre of blood vessels. Saying the cycle “stimulates circulation” describes that immediate response. Saying it is cardiovascular training, prevents heart disease or replaces exercise is a leap the data does not license.

Regular sauna use has its own promising literature, but a systematic review noted that most trials were small and that quality data on frequency, adverse effects and clinical groups is still missing. The possible effects of sauna cannot be transferred automatically to contrast therapy.

“Boosts immunity”The 2025 review on cold water immersion found no significant immediate effect on immune function. That is different from saying the practice causes low or high immunity. It means the claim is bigger than the evidence.
“Speeds up metabolism and burns fat”Feeling cold raises energy expenditure acutely. There is no proof that a contrast routine, without changes to food and activity, produces meaningful fat loss.
“Releases dopamine for hours”The body responds to cold with neurochemical changes, but turning that response into a number, a guaranteed duration or a mood treatment is marketing, not clinical advice.

The problem is not enjoying the circuit. It is selling a bathing experience as though it were a treatment for anything. A bathhouse can offer recovery, presence and pleasure without promising to cure anxiety, depression, metabolic disease or injury.

Who should be more careful with contrast

Abrupt cold asks a lot of the cardiovascular system. The American Heart Association warns that immersion can rapidly raise breathing, heart rate and blood pressure, and recommends particular caution for people with a cardiac history. That is not a detail to skip at a bathhouse front desk.

Anyone with heart disease, uncontrolled blood pressure, a history of fainting, arrhythmia, or a condition affecting the response to heat and cold should talk to the health professional managing their case before getting into an ice tub. The same goes for anyone pregnant, taking medication that affects blood pressure or heart rate, or recovering from illness or injury. Alcohol and contrast do not go together either.

For everyone else, a good venue makes the rules visible, encourages hydration, lets you get out at any moment, and does not treat time in the cold as a test of courage. Someone who leaves early has not “lost” the session. They listened to their body.

How to choose a place, and use common sense

If you want to try it, look for a space that explains the circuit without promising miracles. Ask how getting into the cold works, where you rest, and whether staff are on hand to guide beginners. The quality of the experience shows up in those answers, not in the lowest number on the thermometer.

Contrast therapy is one part of bathing culture, not its whole definition. The heat, the water, the rest and the conversation still count even when nobody is measuring performance. And the best version of the ritual may be the quietest one: get in, feel it, get out before the excess, and go back to your life with more room in your head.
